Physiological characteristics of upper airway obstruction

Specific conductance and flow volume curves were analyzed in 10 normal subjects and 10 patients each with early extrathoracic upper airway obstruction and chronic obstructive lung disease. Upper airway obstruction was characterized by decreased peak flow and specific conductance in spite of normal FEV1 (forced expired volume in 1 s) and MMFR (maximum mid-expiratory flow rate). The ratios .ovrhdot.V50 Exp/.ovrhdot.V50 Insp. [50% vital capacity], PEFR (peak expiratory flow rate)/.ovrhdot.V50 Exp. and PEFR/MMFR were most useful in distinguishing the 3 categories sobjects. PEFR/MMFR could be easily obtained without sophisticated instruments and is helpful in detecting early upper airway obstruction.
