Hope you’re doing well today! I am sorry to hear about the issues you’re facing.
I was able to replicate the issue on your form, however, I could not notice any console error that could point to any specific issue. I also tested a form on my test site and it was submitted fine. It means that the issue could be specific to your site.
Such issues may arise due to possible conflicts on the site due to plugins/themes. In this case, to narrow down the issue further, would it be possible for you to conduct a full conflict test?
To perform a conflict test, please make a full backup of your site as a safety checkpoint and deactivate all the plugins except Forminator and switch to the default 2024 theme and check if the form works fine. If it does it means that there could be a possible conflict.
Further, you can activate each plugin at a time and check until the issue is replicated. The last plugin you activated before the issue was replicated is the plugin that could be conflicting with Forminator. Similarly, if the issue is not replicated after activating all the plugins – you can switch to the currently active theme that you’re using and check if the issue is replicated.

However, if the issue is still replicated with all plugins inactive, we would need to take a closer look at your form to replicate the issue on our lab site and troubleshoot it further. For this, you can follow the below steps to share the form export with us.
To share the form export
– Navigate to the “Forminator -> Forms” page in your site’s back-end
– click on a little “gear” icon next to the form in question
– select “Export” option
– copy given export code and paste it at https://pastebin.com
– include link to that paste in your response below.
Note: this export will not include any data that is already submitted – they’ll stay safely on your site and we don’t need them anyway.
