It starts with Guybrush stranded on Grogg Island after a shipwreck. He meets Commander Keen (whose spaceship also crashed on the island), and who tells him that LeChuck teamed up with Mortimer McMire and Purple Tentacle to build a giant Destructomatic T-47 at the Heart of the Universe to destroy it. To do this, they need to collect four crystals, and Guybrush and Keen join forces to find these crystals first...

The game provides examples of:
- Adaptational Explanation: The first chapter of the game explores what happened to Guybrush at the Carnival of the Damned between Monkey Island 2: LeChuck's Revenge and The Curse of Monkey Island. It's even titled "The Secret Revealed or your Money Back" (Ron Gilbert had previously stated that if he made another Monkey Island title revealing the Secret, he would have called it Monkey Island 3a: The Secret Revealed or your Money Back).
- Amusement Park of Doom: The first part of the game takes place in the Carnival of the Damned.
- Legion of Doom: LeChuck, Mortimer McMire, and Purple Tentacle.
- My Future Self and Me: In the fifth chapter "Me, Myself, and Guybrush", Guybrush travels in time and encounters versions of himself from the past.
- Shout-Out: The name of the fifth chapter "Me, Myself, and Guybrush" is a reference to the movie Me, Myself & Irene.
- Space Pirates: LeChuck becomes one, having teamed up with the intergalactic villain Mortimer McMire.
