Much like the first game, the world of Fear & Hunger doesn't shy away from the disturbing and horrifying content. And just because it takes place in a time period more modern than the first games doesn't mean the world is any less horrible.- The concept of the Festival of Termina as a whole is pretty horrible right off the bat, being a town-wide free-for-all in which everyone in town participates... willingly or otherwise. Think of it like The Purge, but it's all day long until only one person remains. And the one in Termina is different, as it has been taking place for some time prior to the Contestants arriving.
- And this isn't even the first Festival of Termina, but rather the third. The first happened sometime prior, which involves all the villagers going mad and killing each other. But when the first Festival ended in failure, the survivors were Moonscorched into horrible abominations. The second started when the Bremen Army invaded, once again causing a massive town-wide free-for-all, with only a single survivor (that being a Bremen Soldier found in the Ruined Streets) The third Festival starts when the arrival of the 14 Contestants in Prehevil.
- And speaking of the Soldier, talking to him has him state that while his skin is fine, his mind is rapidly going as he's fully embraced the Festival, all while he also seems to be aware of the 3-day time limit of the current Festival. Which indicates the second Festival is still technically ongoing at the same time as the third.
- And this isn't even the first Festival of Termina, but rather the third. The first happened sometime prior, which involves all the villagers going mad and killing each other. But when the first Festival ended in failure, the survivors were Moonscorched into horrible abominations. The second started when the Bremen Army invaded, once again causing a massive town-wide free-for-all, with only a single survivor (that being a Bremen Soldier found in the Ruined Streets) The third Festival starts when the arrival of the 14 Contestants in Prehevil.
- Worse yet, during the Festival, the villagers have all become Moonscorched. What it is is vague and unknown, but it starts with severe burns and cracked skin. All the while, the afflicted can feel something underneath. As it progresses, the sufferer may begin to transform and mutate, as seen with the likes of the multi-limbed Bobbies or the one-legged Rat Hag. Regardless, almost all are driven to murderous insanity, driving them to kill outsiders on sight.
- If you go into the sewers and manage to talk to the Moonscorched villagers, they'll mention that during the first Festival, some managed to retain their sanity and not kill their loved ones. But in doing so, they were punished by becoming Moonscorched and are now in constant pain, to the point of attacking outsiders on sight.
- Perhaps the more unnerving thing about Moonscorching is what is implied about it from the lore. Per the information given about Rher, the god allegedly reveals truth under the moonlight, but per a skin-bible written by Enki, what Rher does is strip away the truth and leave only a lie. Given what each of the main cast's Moonscorched forms look like, the latter is closer to reality. So, ultimately, a distant god strips away everything true about yourself, and forces you to become a lie that has nothing to do with who you are. Small wonder those who undergo moonscorching go mad.
- Rather early in the game, you can encounter the Woodsman killing a random villager before turning his attention towards you. Trying to talk to him has him tell the player to leave, though he's still perfectly willing to kill the player.
- But if you jump down the nearby well, he'll give you a rope to climb back out (but not before killing Tanaka), where he'll be scarily lucid, warning the player to leave. Only once will he give them the option to leave, as interacting with him again will make him attack the player.
- Bobbies themselves aren't uncommon enemies to fight, if a bit unorthodox. But they have a unique mechanic in which the player has to stomp on them after defeating them in battle, otherwise they'll get right back up. Inattentive players or those who just don't know will be in for a surprise when a Bobby suddenly gets back up after being defeated.
- In the Book Store, a Bobby can suddenly enter the building. Unusual, as they're mostly encountered outside. If the player tries to sleep in the upstairs bed without killing it, the Bobby will ambush the player while they try to sleep.
- And the Contestants aren't much better. Each of them can succumb to Moonscorch at different times, seemingly tied to their own mental well-being, with the player being able to resist until the end of Night 3, and judging by most of their reactions, the transformation is anything but pleasant. Not to mention each one's transformation seems indicative of who they are or their personal fears.Game Over text via Moonscorch: "You are feeling horrible... Your body aches like never before... Something is moving inside you... Something primal is trying to get out of your hollow shell... Your body... It's bursting at its seams!"
- Abella transforms into the monstrous Chaugnar, a large, elephant-monster with freakishly terrifying strength. Worse is that it appears by bursting through a wall in Tunnel 7, with loud banging signaling its arrival. And if you manage to evade or defeat it on Level 1 of Tunnel 7, it'll just appear again Level 2 once you take the elevator, where it'll give chase again until it's either killed or the player escapes through a Rher Sigil. It can also be found in Tunnel 6 if it hasn't been killed prior.
- Disturbingly, performing Medical Diagnosis on Chaugar after defeating it states that it's still alive.
- Henryk becomes the polite yet intimidating Gentleman, who acts polite and refined, but eats vulgar food that resembles genitalia and can kidnap Marina.
- Caligura becomes the appropriately named Monster, lurking around the sewers before rising up to attack. The Monster looks like a horrible giant fusion of testicles with a vagina-shaped mouth lined with teeth, all with countless arms
- Levi transforms into the Weeping Scope, a humanoid creature with its face morphed into a giant scope similar to that of a gun, firing at others while crying.
- O'saa transforms into the Mastermind, a creature resembling the Mumbler enemies from F&H1 that appears at Mausoleum Alley in the overworld and casts Hurting if the player gets too close, much like the Yellow Mages from the first game.
- Olivia's Moonscorch form is the Mechanical Dance, a horrifying human-machine hybrid that wanders the Museum. She proclaims she's "always been like this" and begs the player to not look at her, seeing herself as an abomination.
- Marina turns into the Cocoon, with her body being horrifically morphed causing her head to emerge from between her legs, while a structure from where her head used to be controls the main body. Despite not wanting to fight, a figure she identifies as "Father" commands her to do so.

- Within Prehevil, you'll have to go through the Department Store to reach the Orphanage. However, upon entering, there's no background music. The silence is quickly interrupted by blood-curling screams of agony in the distance; screams that are always heard, regardless of where you are in the store. Climbing up the stairs causes the screams to get louder until you reach the top and find several people hanging from chains attached to their bodies; some even have flayed skin. There's no way to interact with them, and you have to go through the store to travel between the Prehevil - Shopping Street and Prehevil - North-West areas. That the only item on this floor, a Leechmonger ring, is found on a corpse sitting at the end of the walkway doesn't help, either.
- While exploring the town, you can come across several coffins. At first, they just seem like background elements and you won't think much of them. But due to the backtracking within the game, passing by certain coffins three times can cause a Death Mask to climb out and give chase. Death Masks themselves being powerful enemies that can cut off a limb with little issue and have a lot of health. When playing on Fear & Hunger difficulty, two Death Masks will always spawn from two out of six randomly selected coffins in each playthrough. But on Maso Mode, all six coffins will have the Death Mask.
- The White Mold Apartments feel like something out of Silent Hill, with the Neighbors wandering around and the droning ambient music playing throughout. What's worse is that a mass murder occurred in the apartments, with a man killing certain residents within the building. This information is important to access the Rher Dimension version of the Apartments. Some of the rooms also have beds that give the option for the player to sleep on them, which the game advises against sleeping. Doing so causes the player character to fall victim to the white mold and never wake up, causing a game over.
- And speaking of the Rher Dimension version of the Apartments, the area is filled with Neighbors, as well as various decapitated people sitting in chairs as something is being pumped out of them. (It's possible that they were the victims of the serial killer, or at least morbid effigies thereof.) And the music, Relentless
, makes the area all the worse, with the sounds of a man screaming combined with something playing alongside it, bringing to mind "My Heaven
" from Silent Hill 1.- And depending on how things go, you may end up having to encounter one of the Moonscorched Contestants in the Rher Dimension version of the Apartments. If Marcoh wasn't recruited or killed by Day 3 Morning, the Giant will appear in one of the cells, only to escape after some time, where he'll wander the Dimension. The longer the player stays on a floor, the more likely he'll appear on said floor. And if Tanaka is still alive by Day 3 Morning, he'll appear as Judgment, also wandering the Rher Apartments.
- Thankfully, it's impossible to encounter both Giant and Judgement in the Apartments. Though if Marcoh Moonscorched while Tanaka is still alive, you can encounter a shaken Tanaka commenting on Marcoh's transformation. If found in the White Mold Apartments on Day 3 while Marcoh is recruited or dead, he'll undergo a massive personality change and become hostile to the player, hellbent on murdering them.
- And depending on how things go, you may end up having to encounter one of the Moonscorched Contestants in the Rher Dimension version of the Apartments. If Marcoh wasn't recruited or killed by Day 3 Morning, the Giant will appear in one of the cells, only to escape after some time, where he'll wander the Dimension. The longer the player stays on a floor, the more likely he'll appear on said floor. And if Tanaka is still alive by Day 3 Morning, he'll appear as Judgment, also wandering the Rher Apartments.
- Needles more or less serves the same role as the Crow Mauler from the first game. He can first be encountered in Tunnel 7, in the process of cutting off Tanaka's head before turning his attention to you, in which he'll chase you down. If you don't trigger the scene with him and Tanaka or manage to avoid Needles, he'll appear in various locations within the city, signaled by his laughter and a message saying "You hear distant laughing. It sounds cold and devoid of life..." In each area, Needles has a chance of spawning the longer you stay in said area, at which point he'll try and ambush the player, either hiding behind a building to jump out when you get close or suddenly appearing from off-screen to give chase.
- Needles himself is an infamously difficult enemy for so early in the game due to his high health and his ability to inflict status effects such as bleeding and infection. Want to take out a limb? Well better take them both out or he'll pull out a gun with his undamaged arm and deal hefty damage. Also, both arms also have a high amount of health.
- And the first time he defeats you, he won't kill you. Instead, you'll just be chained up in Tunnel 5 until some shadowy figure approaches and gives the character a "massive orgasm" until they pass out. When they wake up, they'll be freed. However, any characters that were in the party will be removed and their remains can be found throughout the city.
- Marina's head can be found on a fence in Prehevil - Central, located in front of the Book Store with her body being in a nearby alley. Levi's will be in the same place. And in as a morbid joke, if both of them were in the player's party, their heads will be facing each other, as though kissing.
- Abella's body is found at the Prehevil - Staircase, where the path splits to a staircase or a ladder and wooden plank. She's crucified on a wall with blood streaming down her body.
- Marcoh can be found in the Sylvian Square, northeast of the statue, face-down in a pool of his own blood.
- Daan is at the News Agency, near the bottom-right of the map, behind a car with knives in his back.
- O'saa is located at the Temple Site (Tower) next to a lamp in the middle of the street, minus his arms and legs.
- Karin is found in front of the Church. Perhaps most tragically, she's still alive when you see her, but just barely as she crawls to you, only to die from her injuries in front of you.
- Olivia's lower half can be found at Prehevil - North-West, near the stairs leading the News Agency. Specifically, it'll be left in her wheelchair, with an arm and her head scattered nearby.
- Depending on the situation, Needles may have some sort of macabre trophy each time you fight him. If you encounter him in Tunnel 7 after he killed Tanaka, he'll be wearing his hat. If Marina hasn't been recruited by Day 2 Evening, Needles will remove the skin from her face and wear it over his own like a mask. It's even possible to find Marina's body at the book store with the skin flayed from her face.
- And similar to the aforementioned Needles, the Gull Bros (who only appear on Maso Mode) will appear after certain conditions are met (either getting 2 of the 3 effigies or if you don't kill them after using the Tes'tich table in the Orphanage) and will roam the city in search of the player. Their arrival in an area is heralded by the sounds of seagulls, a message saying "A terrifying presence has entered your vicinity" and the ever-familiar horn that played when the Crow Mauler appeared in the first game.
- And lurking on the first level of Tunnel 1 is Stitches, who is implied to have created the Sewjobs and certainly created the Living Flesh that also haunt the first level of Tunnel 1.
- Losing to Stitches gives the player a unique death animation in which they're sewn together as part of a Living Flesh, a fate probably even worse than the Human Centipede.
- And speaking of the Sewjobs and the Living Flesh, they're both amalgamations of flesh fused together by unknown methods. Living Flesh is just, well, a lump of living flesh while the Sewjobs seem to be comprised of arms to form the umbrella and various heads that make up its body. Talking to the Sewjobs reveal they're perfectly sentient and capable of speech. Not only that, but they consider their life to be better than what they were before, with each head having their own identity. And if trying to tell them they don't need to fight, they reply that everyone has to fight under the moon and that only by joining together can a bloody fate be prevented.
- Poes are somewhat rare enemies that only appear when interacting with the lamp in Dr. Kefer's Tricks & Magic store, which will cause Poes to spawn from various radios throughout Prehevil. If the player is unprepared or not expecting them, they can really get the jump on them. They're also not particularly tough, as they only have a Coin Flip attack, which can easily be avoided by having the main character guard while the teammates attack. But fighting them in the first place yields nothing other than stealing a Soul Stone from them. In Maso Mode, all Poes are active and will give chase if they spot the player. Should you lose to them, they'll repeatedly smash you against the floor by your ankle and then fist you.
- Doppelgängers can be encountered throughout the town. At first, they seem no different from the other Contestants, but they show unusual oddities. The first one the player will most likely encounter would be Tanaka's in Tunnel 4, where he's facing a wall; talking to him will have him turn his head a full 180° to face you. Olivia's can be seen running around the Deep Woods, literally running on her own two feet while the real Olivia is bound to a wheelchair. Marina's is the most normal, being encountered in the Book Store (the same place where you can recruit the real Marina), but she'll start acting strangely if you question her for too long. The sole exception is Levi's in the sewer, as the moment you get close, he'll run to you and attack.
- When fighting one of the Doppelgängers, they'll loudly proclaim they'll "SWALLOW YOU WHOLE" with their battle portrait showing them with an insanely wide mouth over their face, as well as no eyes. Marina's, however, has its mouth diagonally across its face, rather than horizontal.
- Levi's Doppelgänger seems to be the least complete, being the only one that's aggressive (the others won't attack until you do) and visibly mutated on the overworld; its battle portrait is similar to Levi's, but lacks fully detailed hands.
- And crossing into Fridge Horror, there's no explanation as to why these Doppelgängers exist. The concept of a Doppelgänger as a whole is a creature meant to mimic a person and take their place with it considered to be a bad omen if you see your own Doppelgänger. But these have no explanation as to why they're here and what their purpose is supposed to be for. Even when meeting the originals, most don't acknowledge them; only Marina and Olivia do, expressing confusion and anger.
- Though Rancid the Sergal implies they were created specifically to replace the passengers so no one would suspect they were missing.
- In Marina's Ending B, Marina is seen with a girl who strongly resembles Samarie. However, Samarie is supposed to be dead; one of the most common explanations is that it's Samarie's Doppelgänger.
- You don't fight Karin's Doppelgänger, but you see it in the White Mold Apartments, hosting a news broadcast where she reports on a murder that occurred in those same apartments. If you're playing as the real Karin or have her in the party, she'll be confused and claimed she never recorded the broadcast.
- There's a new kind of enemy known as the Beekeeper that appears in three specific spots; in two locations (the West Tunnels and Ruined Streets), they drop from their hive if the player gets too close. But for the third, they appear in the Back Alleys, when walking by a certain garage. First time walking through, the text notes that you hear bees. When walking by that same garage, the Beekeeper will suddenly appear and give chase, complete with the loud noise of the garage door opening.
- The Orphanage that Levi grew up in is dark and dismal, full of Shadowy Figures (ghost children) and Fallen Cherubs (Moonscorched children). Even worse, you can find chained up child skeletons and a Decrepit Priest in the basement, with the main floor having loose boards that (if you lose the coin toss) have you fall into the basement. Not to mention the courtyard in the middle has a lone noose. And the boss of the area, Father Hugo, is implied to have used some kind of method to extend his life. Worse still, there's an unused battle sprite for a naked Hugo that has a Cherub fused to him, implied to have been a Marriage. And given how Marriages are performed...
- And if Levi has Moonscorched into the Weeping Scope, he'll fire at you while you traverse the hallways, making things even harder.
- The Rher Dimension version isn't much better, being reminiscent of the trenches in a war zone with Ronteals that give chase when you get close to their holes. And if you didn't kill the Weeping Scope in the Orphanage, he'll suddenly appear and start chasing you down until you leave.
- The White Bunker itself is already an unnerving area, filled with powerful Bremen soldiers including Flametroopers that can set you on fire if they see you. And yet, also in the bunker is some bizarre Marriage between a human and a crow, as if some perverted recreation of the Crow Mauler from the first game. Strangely, interacting with it does nothing and the characters don't comment on it.
- The Nights can be regarded as one of the worst times of the day, due to poor visibility and draw distance around the player. Not only is the screen darker, but you also can't see as far as Day or Morning, making it easier for enemies to sneak up on you. Made even worse on Maso Mode, in which you start on Day 3 Night with all Contestants being Moonscorched and with difficult enemies running around, including a Platoon in the Ruined Streets.
- If you thought the Centaur was horrible, there's an unused battle sprite
that seems like an even more unholy fusion, complete with the back head having Needles' face. - The Museum is a rather unnerving place. At least the place doesn't have any 'normal enemies, but the background music is tense and foreboding. Not helping things is how dim the interior is, regardless of the time of day, and the Museum itself, despite being somewhat small, can be disorienting, especially with how it loops around. The building is full of people, but all of them seem to be in some sort of autopilot-like trance, each of them talking and moving their heads in unison, but no words being spoken. And wandering around the building is some kind of... thing with a white sheet over its body and wearing a crown. Interacting with it does nothing, as it just wanders around aimlessly and whatever it looks like is entirely unknown.
- But the Museum is not without its dangers, as the Mechanical Dance and Judgement, the Moonscorched forms of Olivia and Tanaka can be found wandering the Museum, both on Day 3. And if Daan wasn't dead or in your party, he'll Moonscorch into Pocketcat (potentially being responsible for Olivia Moonscorching if she wasn't in your party) and can also be found in the Museum. And if you're unlucky (or playing on Maso-Mode), all three can be encountered on Day 3 Night, with Mechanical Dance and Judgement all roaming the area
- It's possible to use Mind Read on Pocketcat (Daan) in battle, where he reveals he's fully aware of you peering into his mind, even taunting you for doing so.
- Not only that, but the Museum also has Poes. Yes, multiple Poes, since all other areas where they appear just have one. The Museum has four of them and if you messed with the lamp in Dr. Kefer's Tricks & Magic shop, then those Poes will become active. And if that's the case or playing on Maso-Mode, you'll have to deal with the Poes and the Moonscorched Contestants.
- But the Museum is not without its dangers, as the Mechanical Dance and Judgement, the Moonscorched forms of Olivia and Tanaka can be found wandering the Museum, both on Day 3. And if Daan wasn't dead or in your party, he'll Moonscorch into Pocketcat (potentially being responsible for Olivia Moonscorching if she wasn't in your party) and can also be found in the Museum. - After saving Samarie and preventing her from Moonscorching, you can take her back to the Train, where she'll remain non-hostile. If you play as anyone except Marina and have the Mind Read ability, your character can read her mind; she'll immediately realize that you're doing this and angrily tell you to stay out.
- Caligura. Unlike the more fantasy forms of horror he's a normal man... and also a creep who can either attempt to attack or sexually assault the player depending on the route. Luckily he's not the hardest foe to fight, but he's still deeply unpleasant to encounter.
- The player can become this if they decide to hunt down and systematically kill all the other train passengers.
